According to the Libertyville Police Department the accident scene was at Route 137 between Cass Avenue and Bell Lane. First responders began to go to the accident scene at approximately 10:52 a.m.

The Libertyville Fire Department and Mundelein Fire Department had to extricate both drivers, including Peshut, from their vehicles due to the heavy damage.
Peshut was pronounced dead at the scene and the other driver sustained non-life-threatening injuries.
Paramedics transported the other driver to Advocate Condell Medical Center in Libertyville for treatment.
No one else was injured in the collision. Police have not released any details on how the crash occurred.
The Lake County Major Crash Assistance Team was called to the scene to assist the Libertyville Police Department with the investigation.
